Agreement Form: By submitting this form, I am providing my digital signature agreeing that may email me or contact me regarding services by telephone and/or text message utilizing automated technology or a pre-recorded message at the telephone number(s) provided above. I understand this consent is not a condition to purchase services or other goods. Calls may be recorded for quality assurance.
Acknowledgement of Risks and Responsibilities: I understand that there are inherent risks associated with training, boarding, socializing, walking, and working with dogs. I assume any and all responsibility for myself and my dog(s) during these activities, provided due diligence and care have been exercised. I will not hold Wyma-Bradley Dog B&B, Amanda Wyma-Bradley, Jared Wyma-Bradley, or any affiliates or property owners responsible for any injuries or misfortunes (to or by me or my dog) during these activities, should they occur.
Health and Care Acknowledgement: I understand that my dog(s) may interact in ways that could result in cuts or scratches, and that my participation in these activities carries risks such as tripping, falling, getting knocked down, jumped on, bitten, or scratched. Additionally, I acknowledge that my enrollment in Wyma-Bradley Dog B&B programs relies on my representation that my dog(s) are in good health and have received necessary vaccinations.
Emergency Situations: In case of injury or illness, Wyma-Bradley Dog B&B may contact my designated veterinarian. If the veterinarian is unavailable, Wyma-Bradley Dog B&B is authorized to take my dog(s) to the nearest animal hospital for immediate care. I agree to reimburse Wyma-Bradley Dog B&B for any medical expenses accrued during their supervision, releasing them from responsibility regarding decisions made by the veterinarian.
Liability Release: I release and agree to save and hold harmless Wyma-Bradley Dog B&B, Amanda Wyma-Bradley, Jared Wyma-Bradley, any affiliates, or property owners from any and all liability, claims, suits, actions, loss, injury, or damage caused by my dog(s) during training, boarding, socializing, or walking. I indemnify Wyma-Bradley Dog B&B for any such liability, claims, suits, actions, losses, injury, or damage.
Financial Responsibility: I acknowledge sole financial responsibility for any damage or harm caused by my dog during the aforementioned activities.
Media Release: I consent to the use, posting, and advertisement of any picture, video, or recording taken during boarding, walking, or training sessions by Wyma-Bradley Dog B&B.
Individual Responsibility: I recognize the need to ask questions and use my own common sense, retaining the right to decline any action that makes me uncomfortable.
Payment Terms and Conditions: I agree to all payment terms and conditions outlined in email correspondence and on Wyma-Bradley Dog B&B’s website.
Media Usage Consent: I release any rights to media content (pictures, video, audio) for Wyma-Bradley Dog B&B to use in any capacity, personal or commercial, on social media or otherwise.
Confirmation: I certify that I have read and understand the rules and regulations set forth in this agreement and agree to abide by them. I confirm the truthfulness of the contents of this release form and agree to all payment requirements stipulated on the Wyma-Bradley Dog B&B website or in written or email communications.
